For over two decades, Leapfrog has published the biannual Hospital Safety Grade, giving patients a transparent, evidence-based snapshot of how hospitals perform on safety metrics.

How Leapfrog Determines Hospital Safety Grades

Leapfrog releases its grades twice a year—once in the spring and again in the fall. Participation is voluntary, but hospitals that opt out may raise concerns for patients doing their research.
Hospitals are graded on an A–F scale, based on dozens of evidence-based measures grouped into five main categories:

- Surgical problems
- Hospital-acquired infections
- Preventive practices for medical errors
- Performance of doctors, nurses, and hospital staff

Fall 2025 Highlights: NYC and Beyond
Nationally, just over 32% of hospitals earned an “A” grade in Fall 2025.
New York State saw 23% of hospitals earn an “A” grade, moving up one ranking since the Spring from 31 to 30.
New York City continues to see improvements in hospital safety, with more facilities achieving top grades than in previous years. Despite this progress, a number of hospitals remain in the lower grade range, highlighting areas where patient safety remains a concern.
