NYC Hospital Ranking Best/Worst 2025 NYC

Preventable medical mistakes remain one of the leading causes of death across the United States. According to research from Johns Hopkins Medicine, more than 250,000 patients die each year due to avoidable medical errors. Hospitals must be held accountable for failing to maintain patient safety when these mistakes lead to serious harm or fatalities.

Despite increased awareness of safety protocols and best practices, some hospitals continue to fall short in delivering high standards of care. That’s where The Leapfrog Group, an independent watchdog organization, comes in. For over two decades, Leapfrog has worked to shine a light on healthcare facilities through its biannual Hospital Safety Grade reports, giving patients access to transparent, evidence-based evaluations of hospital safety.

In this blog, we’ll discuss the newly released Spring 2025 Leapfrog Hospital Safety Grades for New York City, explain how the grades are calculated, and why these ratings matter when choosing where to receive care.

How Does Leapfrog Determine Hospital Safety Grades?

The Leapfrog Hospital Safety Grade is widely regarded as a national benchmark for hospital safety. Its goal is to help facilities identify and correct dangerous practices and encourage transparency and accountability.

Leapfrog releases its grades twice a year—once in the spring and again in the fall. While participation is voluntary, deciding to opt out should be a red flag for patients conducting their research.

Hospitals are graded on an A–F scale, based on 28 evidence-based measures that are grouped into five categories:

  • Safety issues
  • Surgical problems
  • Hospital-acquired infections
  • Preventative practices for medical errors
  • Performance of doctors, nurses, and hospital staff

Each measure contributes to an overall safety score translated into a final letter grade, giving the public a clear picture of how their local facilities are performing in avoiding harmful errors.

New York Spring 2025 Hospital Grades: Steady Improvement

New York State’s hospital safety performance is on a steady rise. In the most recent 2025 Spring rankings, the state rose to #31 (up from #34 in Fall 2024), with 25.2% of hospitals earning an “A” grade, compared to 22.1% in the previous report.

New York City saw an improvement in overall hospital safety performance as well. Of the 44 hospitals evaluated, nine received an “A” grade, a significant increase from just two hospitals in 2024 and three in 2023.

While many facilities still fall within the “C” range, the rise in top-performing hospitals signals progress. However, several hospitals—particularly in Brooklyn and Queens—continued to receive “D” and even “F” grades, highlighting areas that still require urgent attention.
